A novel method using a single lumen tube and extraluminal bronchial blocker for one-lung ventilation in severe tracheal stenosis: a case report

چکیده
: Double-lumen tubes (DLT) and bronchial blockers (BB) are usually used to functionally isolate the lungs during thoracic surgery. However, for patients with tracheal stenosis, management of one lung ventilation (OLV) in anesthesia is still full challenges due mismatching between trachea lumen tube diameter. In past, a small single-lumen (SLT) combined an endobronchial pediatric BB or extraluminal detached uninvent obtained successful OLV stenosis. Additionally, nonintubated surgical pneumothorax may work. We first report interesting case 65-year-old man history upper left lobe nodule tracheotomies. A chest computed tomographic (CT) scan showed middle was severely narrowed. minor SLT acquired optimal collapse lung. He accepted video-assisted thoracoscopic lobectomy under general anesthesia. After both were removed, patient did not present dyspnea airway injury. The recovered well discharged from hospital week after This method, BB, convenient sputum suction fiber optic bronchoscope examination, moreover, it option severe stenosis cases.

One-lung ventilation in children using the single-lumen tracheal tube.
In children, it has been shown that the angles of the tracheobronchial bifurcation total approximately 80 degrees; the overall mean of the right bronchial angle is about 30o, while the left bronchial angle is about 50o. This tracheobronchial relationship may explain why the tracheal tube is more likely to enter the more vertical and wider right main stem bronchus than the more obliquely placed ...
متن کاملDouble-lumen endotracheal tube for one-lung ventilation through a fresh tracheostomy stoma: a case report.
For one-lung ventilation in many surgical cases, double-lumen endotracheal tubes are the first-line choice for airway management. For most cases, double-lumen endotracheal tubes are faster, easier to place, cheaper, and less prone to malposition than today's bronchial blocker devices. This report describes an elective tracheostomy and the use of a double-lumen endotracheal tube directly through...
متن کاملLeft mainstem bronchial rupture during one-lung ventilation with Robertshaw double lumen endobronchial tube -A case report-
Lung separation using a double-lumen endobronchial tube is necessary for video assisted thoracoscopy (VATs). Bronchial rupture after intubation with a double-lumen endobronchial tube has been rarely reported. We report a case of a 70-year-old man who had solitary pulmonary nodule in his right upper lobe. He was intubated with a left-sided Robertshaw double-lumen endobronchial tube. He underwent...
متن کاملComparison of a modified double-lumen endotracheal tube with a single-lumen tube with enclosed bronchial blocker.
This study compared the modified BronchoCath double-lumen endotracheal tube with the Univent bronchial blocker to determine whether there were objective advantages of one over the other during anesthesia with one-lung ventilation (OLV). Forty patients having either thoracic or esophageal procedures were randomly assigned to one of two groups. Twenty patients received a left-side modified Bronch...
متن کاملRandomized clinical trial comparing double-lumen tube and EZ-Blocker for single-lung ventilation.
BACKGROUND In several clinical situations, lung separation and single-lung ventilation (SLV) is essential. In these cases, the double-lumen tube (DLT) is the most widely used device. Bronchial blocker such as Univent or Arndt Blocker serves as an alternative. The EZ-Blocker(®) (EZ; AnaesthetIQ B.V., Rotterdam, The Netherlands) is a new device promising to exceed clinical performance of DLT. The...
